Creating and uploading a new survey to mFieldwork Web


  1. From your homepage in the Surveys Linked to You panel, click FormBuilder. You will be directed to the FormBuilder landing page. See Figure 1.

2. On the Survey tab,  click Upload XML and in the dialog box that follows. click 'YES'



             



3.Navigate to the XML File you have saved and select Open


4. Fill in the following attributes about the survey. * denotes mandatory

    • Language* – The language that the survey is written in.
    • Name of Survey* – This is the name of the survey, as it will appear in the mFieldwork Web Dashboards and will be the default file name of exported data. The name should be the same as the ODK Form and XML file. NOTE: The name of the survey on phones and tablets comes from the name of the ODK Form that is inserted in the code of the XML Export file. It is not the physical name of the file.
    • Survey description* – This is a small description of the survey.
    • Assign a Location* – This is the geographic location where the survey will be conducted. The list is generated from Locations entered during your initial set-up of mFieldwork Web. See article on Creating, editing, deleting Locations.
    • Assign a Department* – This is the Department focus of the survey. This list is generated from Departments entered during your initial set-up of mFieldwork Web. See article on Creating, editing, deleting Departments.
    • Assign Supervisor* – This is the supervisor assigned to the survey. The list is generated from users entered during your initial set-up of mFieldwork Web. See article on Creating users and fieldworkers and article Managing user roles for a table outlining the Roles and Permissions for Survey Supervisors.
    • Assign Fieldworkers* – These are the people that will be given permission to download the survey onto their phone or tablet. The list is generated from fieldworkers entered during your initial set-up of mFieldwork Web. See article on Assigning fieldworkers to a survey
    • Sampling Methodology – This is information on the sampling methodology for the survey.
    • Set Survey Status – This is the work status of the survey. For a description of Statuses see article Statuses.

5.   When finished completing the fields, click Save & Exit.
